your mind doesn't actually wake up until you swallow some food. if you see sunlight, your mind prepares itself to wake up, but doesn't actually wake up until you eat.
and your voice is lower because everything is still very relaxed, including your voice box.

that's relatively irrelevant.
1) test taking doesn't require an awake mind. you have to remember things, and follow formulas, but you do't have to create your own formulas or adapt to new systems. the most mentally strenuating thing a test can make you do is apply memorized systems to un-thought-of situations. and while that requires a little bit of awake thought, it doesn't require enough to really say that you have to be awake to get an A.
2) most kids constantly do that, and their bodies basically "get used to it". they get used to performing mental work while not really awake. it's analagous to exercising and dieting at the same time.
3) i'm guessing it's possible to force your mind awake. i don't see any reason why not. it'd probably require somewhat extreme circumstances, but those are possible with things like tests.
